A Welcome Letter to Our Patients
Dear Patients,
Welcome to Legacy Rheumatology!
We are honored that you have entrusted us with your care. Our goal is to provide compassionate, thorough, and personalized rheumatologic care to every patient. Because many of our patients have complex medical conditions and require detailed evaluations, we kindly ask for your cooperation with the following important office policies. These guidelines help us provide the best possible care to everyone.
1. Appointment Times and Waiting Periods
Rheumatology visits often require a detailed review of your medical history, examination, and treatment plan. As a result, appointments may take longer than expected.
Please allow 45–60 minutes for your appointment, as wait times may be longer than expected. We ask that you avoid scheduling other medical appointments immediately afterward. Thank you for your understanding.
We sincerely appreciate your patience as we work to provide each patient with the attention and care they deserve.
2. Missed Appointments and Cancellation Policy
We understand that unexpected circumstances can arise. However, our practice has a long waiting list, and many patients are waiting for an opportunity to receive the medical care they need. When an appointment is missed without sufficient notice, that time cannot easily be offered to another patient who may be waiting for care.
To allow us to serve all our patients fairly, we require at least 48 hours’ notice for appointment cancellations or rescheduling.
The following fees apply to missed appointments without the required notice:
-
New patient appointments: $125
-
Follow-up appointments: $75
Please ensure that a valid credit card is kept on file to cover applicable missed-appointment fees.
We kindly ask that you respect your appointment time and the needs of other patients. Missing an appointment without notice means that a valuable opportunity for another patient to receive care may be lost. Your consideration can make a meaningful difference to someone who is waiting for treatment.
Thank you for helping us make care accessible to those who need it.
3. Medical Records From Other Healthcare Providers
To help us evaluate your condition accurately and avoid unnecessary delays in your care, please bring your medical records from other healthcare providers to your appointment.
Please note that we may not receive your records in time for your visit, even if they have been requested in advance. We encourage you to access your patient portals and download your medical records, laboratory results, imaging reports, and other relevant documents before your appointment.
Having these records available allows us to better understand your medical history and make informed decisions about your care.
Thank you for your cooperation.
4. Insurance Cards and Registration
Please present your current insurance card at the front desk at every visit, even if you have visited our office before.
Insurance coverage and plan details may change, and we need to verify your information to process your visit correctly.
A valid insurance card must be presented before your visit can proceed. Without your insurance card, we may be unable to see you at your scheduled appointment. If you have a new insurance plan, please notify our office in advance.
Thank you for helping our team keep your information accurate and up to date.
We sincerely appreciate your trust, patience, and cooperation. Our team is committed to providing thoughtful, individualized care while respecting the time and medical needs of every patient in our practice.
Thank you for being part of the Legacy Rheumatology family. We wish you good health and hope you feel better soon!
With warm regards,
