These Terms of Use govern your access to and use of the website operated by Legacy Rheumatology LLC, doing business as Legacy Rheumatology (“Practice,” “we,” “us,” or “our”), including any appointment-request, payment, contact, and informational features available through the current website (collectively, the “Website”).

Please read these Terms carefully. By accessing or using the Website, or making a payment through the Website, you acknowledge that you have read, understood, and agreed to these Terms.

If you do not agree to these Terms, do not use the Website.

1. Website purpose

The Website provides general information about the Practice, its healthcare providers, location, office hours, services, policies, appointment availability, and payment options.

The Practice does not guarantee any particular response time for Website communications.

4. No provider-patient relationship through Website use

Accessing the Website, reading its content, submitting a contact form, or making a payment does not by itself establish a provider-patient relationship between you and the Practice or any healthcare provider.

A provider-patient relationship is established only after all required registration, identity-verification, consent, eligibility, and clinical intake procedures have been completed and a healthcare provider agrees to evaluate or treat you.

The Practice may decline to establish or continue a provider-patient relationship when permitted by applicable law.

These Terms govern use of the Website. They do not replace any consent-to-treatment form, financial agreement, office policy, patient acknowledgment, arbitration agreement, or other document that may be presented separately at the Practice.

6. Same-day appointments

Same-day appointment availability may change rapidly and is not guaranteed.

A same-day appointment request is subject to:

• available staffing;

• provider availability;

• the nature and urgency of your condition;

• completion of in-person registration;

• applicable age or consent requirements;

• the Practice’s ability to provide the requested service; and

• other clinical or operational considerations.

If your condition appears urgent or potentially life-threatening, do not rely on same-day Website availability. Call 911 or seek appropriate emergency care.

7. Online payments

The Website may allow you to make a payment related to an appointment or healthcare service.

By submitting a payment, you represent that:

• you are authorized to use the selected payment method;

• the billing information you provide is accurate;

• you authorize the applicable charge; and

• you understand that additional charges may become due based on the services actually provided.

8. Cancellations, missed appointments, and refunds

The Practice may charge a late-cancellation or missed-appointment fee when permitted by applicable law and disclosed to the patient.

Unless prohibited by law, processing fees charged by an independent payment processor may be nonrefundable.

If the Practice cancels an appointment and no services have been provided, any refundable prepaid amount will be addressed in accordance with the Practice’s payment and refund policy.

Nothing in these Terms limits any refund, charge-dispute, or consumer right that cannot lawfully be waived.

9. Insurance and benefit verification

Information provided on the Website regarding insurance participation is general and may change.

Participation in an insurance network does not guarantee that:

• your specific plan is accepted;

• a service is covered;

• prior authorization will be approved;

• a referral is unnecessary; or

• your insurer will pay a particular amount.

You are responsible for confirming coverage, network status, referral requirements, prior-authorization requirements, deductibles, copayments, coinsurance, and exclusions directly with your insurer.

You remain responsible for amounts not paid by your insurer unless applicable law or a written agreement provides otherwise.

These Terms and disputes arising specifically from use of the Website will be governed by the laws of the State of Florida, without regard to conflict-of-law principles.

Unless applicable law requires otherwise, legal proceedings relating specifically to Website use must be brought in a court of competent jurisdiction located in Jupiter, Florida.

This section does not change the law applicable to medical treatment or require a patient to waive a venue, jury-trial, malpractice, consumer-protection, or other legal right that cannot lawfully be waived.

The Practice may update these Terms when its Website, services, business practices, or legal obligations change.

The updated Terms will be posted on the Website with a revised “Last Updated” date. Changes apply prospectively from the stated effective date unless applicable law requires otherwise.

Separate patient forms, clinical consents, financial agreements, office policies, and Notices of Privacy Practices continue to apply according to their own terms.
